Sadly the tops of some very old graves have simply caved in. Sometimes it is the supports on the "underneath" portion of the grave aren't strong enough and / or put in correctly. There is some like that in the Cheltenham Cemetery too. There is some in country cemeteries. Unfortunately some types of headstones get black mould on them. There is safe methods of cleaning them with patience
